Privacy & terms

How this practice handles your information, your rights over your health records, and the terms that apply to using this website.

Website privacy policy

This policy covers serenitypeakcare.com. It does not cover health information created during treatment — that is governed by the Notice of Privacy Practices below.

The short version

This website sets no cookies, runs no analytics, and contains no advertising or tracking pixels. It does not build a profile of you, and it has nothing to sell to anyone. The only information it receives is what you type into the question form and send.

What the website collects

  • Information you choose to send. If you use the “ask a question” form, we receive the name, email address, phone number and message you enter. That form is deliberately not for medical details, and says so directly above the fields.
  • Nothing automatic. No cookies are set. No analytics service is loaded. There is no advertising network, session recording, heat-mapping or tracking pixel of any kind on this site.
  • Server logs. The site is hosted on GitHub Pages. Like any web host, GitHub records standard request data, including IP addresses, to deliver pages and protect against abuse. We do not have access to those logs and cannot link them to you.

Information that does not come here

  • New patient intake. The intake form is hosted by Valant, the practice’s electronic health record system. It opens on Valant’s own website in a new tab. Nothing you enter there passes through this website, and it is covered by the practice’s business associate agreement with Valant.
  • The patient portal. Also operated by Valant, on Valant’s domain.

Third parties this website relies on

  • GitHub Pages — hosting.
  • Google Fonts — two typefaces load from Google’s servers, which means Google receives your IP address as part of that request. It is the site’s only third-party content. The fonts can be self-hosted to remove even that; see the project README.
  • Valant — intake and patient portal, linked to rather than embedded.
  • Spruce — secure, HIPAA-protected messaging with the practice.
  • Google Workspace — the practice’s ordinary email. Convenient, but not a secure channel for medical details; use the secure message or the phone for those.
  • Google — patient reviews are read from the practice’s public Google listing.

How messages to the practice are handled

This website has no contact form. Questions reach the practice one of three ways, and they are not treated the same:

  • Secure message through Spruce — encrypted and HIPAA-protected. This is the right channel for anything personal. These messages become part of your record and are retained under the policy below.
  • Phone — call (973) 755-2887.
  • Ordinary email — convenient but not encrypted end to end. Fine for “what does an evaluation cost?” Please do not send symptoms, diagnoses or medication details this way. If you already have, call the office and we will move the conversation somewhere secure.

Nothing you send is sold, rented, shared with advertisers, or used for marketing, in any channel.

How long records are kept

Clinical records are retained for seven years from the last date of service. That matches the longest minimum required in any state where care is provided: New Jersey requires seven years, New York and Arizona six, and Florida five. Where a patient was a minor, records are held at least until the longer period that state requires has run.

HIPAA documentation, including this notice, signed authorizations and complaint records, is retained for six years, as federal law requires.

Your choices

To ask what information you have sent through this website, to correct it, or to have it deleted, contact the practice at Care@serenitypeakcare.com or (973) 755-2887. Requests about treatment records are handled under the Notice of Privacy Practices below, which gives you stronger rights.

Children

This website is not directed at children under 13 and does not knowingly collect their information. The practice treats adolescents aged 15 and older; a parent or guardian is involved in care for patients under 18.


Notice of privacy practices

THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.

Effective date: July 26, 2026. This notice applies to Serenity Peak Psychiatry and Integrative Wellness LLC.

Our commitment

We are required by law to protect the privacy of your protected health information, to give you this notice describing our legal duties and privacy practices, and to follow the terms of the notice currently in effect. “Protected health information” means information about you, including basic identifying details, that relates to your health, your care, or payment for your care.

How we may use and disclose your information without your authorization

  • Treatment. To provide, coordinate and manage your care. For example, we may share information with a primary care physician, another specialist, or a laboratory performing tests we order.
  • Payment. To bill and collect payment. This practice is private pay and does not bill insurance, so payment disclosures are limited — for example, providing an itemized receipt at your request, or working with a payment processor.
  • Health care operations. To run the practice: quality review, staff training, licensing, audits, and business management.
  • Appointment reminders and treatment alternatives. To contact you about scheduled appointments or to describe treatment options that may interest you.
  • As required by law, including public health activities, reporting suspected abuse or neglect, health oversight activities, judicial and administrative proceedings, law enforcement requests, coroners and medical examiners, organ donation, approved research, workers’ compensation, military and national security, and correctional institutions.
  • To prevent a serious threat to your health or safety or that of another person.
  • To family or others involved in your care, where you agree, or where you are unable to agree and disclosure is in your best interest.

Uses that always require your written authorization

We will not use or disclose your information for any of the following without your signed authorization, and you may revoke that authorization in writing at any time:

  • Psychotherapy notes, except in the limited circumstances the law allows.
  • Marketing, including using your name, words or image in any promotional material. A review or testimonial is marketing.
  • Any sale of your protected health information. We do not sell patient information.

Additional protections

Mental health, substance use, HIV-related and genetic information carry protections beyond HIPAA under the laws of the states where care is provided — New Jersey, New York, Florida and Arizona. Where state law is stricter than HIPAA, we follow the stricter rule.

Your rights

  • Inspect and get a copy of your health record, usually within 30 days. We may charge a reasonable, cost-based fee for copies.
  • Ask us to correct information you believe is wrong or incomplete. We may decline, and will explain why in writing.
  • Request confidential communications — for example, asking us to call only a particular number, or write only to a particular address. We will accommodate reasonable requests.
  • Ask us to limit what we use or share. We are not required to agree, except that we must agree to withhold information from a health plan when you have paid for that service in full yourself.
  • Get a list of disclosures we have made, for the six years before your request, other than for treatment, payment and operations.
  • Get a paper copy of this notice at any time, even if you agreed to receive it electronically.
  • Choose someone to act for you — a person with medical power of attorney or a legal guardian may exercise these rights.
  • Be notified if a breach occurs that compromises the privacy or security of your information.

Our duties

We are required to maintain the privacy of your protected health information, to notify you following a breach of unsecured information, and to abide by the terms of the notice currently in effect. We reserve the right to change this notice and to make the revised notice apply to information we already hold. A current copy will always be available on this page.

How to complain

You may also file a complaint with the U.S. Department of Health and Human Services, Office for Civil Rights, at 200 Independence Avenue SW, Washington DC 20201, by calling 1-877-696-6775, or at hhs.gov/ocr/privacy/hipaa/complaints.

You will not be retaliated against for filing a complaint.

This website is not medical advice

Everything here is general information. It is not a diagnosis, not a treatment recommendation, and not a substitute for evaluation by a qualified clinician. Reading this site, sending a question through it, or beginning an intake does not create a patient–provider relationship. That relationship begins only when the practice accepts you as a patient and care formally starts.

This website is not for emergencies

The website and its forms are not monitored continuously. If you or someone you know is in immediate danger, call 911. For urgent mental health support at any hour, call or text 988 to reach the Suicide & Crisis Lifeline. This practice does not provide emergency or after-hours crisis services.

Where care is available

Telehealth services are available only to patients physically located in New Jersey, New York, Florida or Arizona at the time of the visit. This is a licensing requirement, not a preference, and it depends on where you are sitting rather than where you live.

Results are not guaranteed

Outcomes described for psychiatric care, medical weight loss, NAD+, peptide therapy and hormone optimization vary between individuals and depend on clinical factors specific to you. Nothing here promises a particular result. ADHD testing and treatment are subject to state-specific regulations, and what may be prescribed by telehealth differs between states.

Fees, payment and appointments

This is a private-pay practice; insurance is not billed for any service. Current prices are listed on the services and pricing page and are subject to change without prior notice. Laboratory testing, medications, supplements and compounded prescriptions are billed separately unless stated otherwise. Payment in full is due at the time of service, and is owed regardless of whether your insurance later reimburses you. A superbill is available on request as a courtesy; submitting it and following up with your insurer is your responsibility. See the insurance and reimbursement policy.

Appointments canceled, rescheduled or missed with less than 48 business hours’ notice are subject to a $75 non-refundable late cancellation fee. The full policy is on the services page.

Intellectual property

The text, artwork, logo and design of this website belong to Serenity Peak Psychiatry and Integrative Wellness LLC and may not be copied or reused without permission.

Links to other websites

This site links to services the practice uses, including Valant and Google. Those websites have their own terms and privacy policies, which govern your use of them. We are not responsible for their content or practices.

Changes and governing law

These terms may be updated; the date at the top of this page shows when they last changed. Continuing to use the website means you accept the current version. These terms are governed by the laws of the State of New Jersey.
